Collaboration between PHP and NHS Leeds Teaching Hospital
Personal Homecare Pharmacy Ltd (PHP) and Leeds Teaching Hospital Trust (LTHT) is delighted to announce that their submission has been shortlisted for the 2025 HSJ Partnership Awards 2025, recognising an outstanding dedication to improving healthcare and effective collaboration with the NHS.
Now in their eighth year, the HSJ Partnership Awards, have become the most recognised and respected mark of the strongest relationships between suppliers and the NHS.
Following the thorough judging process, PHP and LTHT was included in 180+ organisations, projects and individuals that made the final shortlist amid 280+ entries logged for the 2025 awards programme.
PHP and LHTC shared an objective to ensure that clinical homecare services designed and offered to patients with Long term conditions (LTC), were done in collaboration and offered new innovative solutions to drive much needed efficient practice. Together we share a passion to deliver clinical homecare services that are safe, cost-effective, timely and designed to ensure seamless integrated patient care for our patients. These services include dispensing and delivery of complex high-cost medicines to patients’ homes, as well as processing and management of prescriptions and invoices. In addition, nursing support can be provided to help patients self-administer treatment or to have treatment administered to them at home.
The solution was to adopt a robust, secure e-commerce platform enabling us to offer patient and NHS portals that provide real-time data to clinicians, offer choices to patients and improve pharmacy working practices in the NHS and PHP. Repurposing this technology enabled PHP to improve the safety and reliability of services and enabled continuous growth, while minimising development and operational costs. We evaluated this with NHS Leeds Teaching Hospital. This Trust aim was to reduce the average working days between when patient registration was received by a Homecare Provider, and when the patient commenced on treatment. Also, they aimed to cut down the number of working days from receipt of patient’s prescriptions to registration, and to their first order for medication being placed. PHP was engaged with the task of improving this service. This was evaluated in over 1500 patients.

The selected winners will be announced during the awards ceremony at Evolution London on 20th March 2025.
